Have you had an annual wellness visit?

The Annual Wellness Visit is a preventative benefit now covered by Medicare FREE of charge. The visit includes one-on-one interaction with our specially trained Annual Wellness nurse, counseling on preventative services you should receive and referral sources for other care you may need. Rather than waiting until you have an illness requiring treatment, the annual wellness visit focuses on helping you maintain and enjoy good health. Please note that the annual wellness visit is not a physical exam.

Karla Kuhn, RN


Karla Kuhn, RN is an Annual Wellness Resource Specialist. Karla has been involved in various areas of nursing since 1990.

She is a 2001 graduate of the I.U. East school of nursing. Karla did case management for Reid Health prior to becoming a part of Reid Internal Medicine.

Karla was a hemodialysis nurse in an outpatient care setting for 13 years. She also served as the Community Health Center Education Nurse Manager and has experience as a parish nurse.

What is an annual wellness visit?

As a current participant in Medicare Part B or a Medicare Advantage Plan, you may be eligible for an annual wellness visit at no charge and without a co-payment or deductible. This is not an annual physical, but a one-on-one visit with a specialty trained registered nurse who will review your medical history and help you plan for a healthy future.

What to expect at your annual wellness visit

During an annual wellness visit, a registered nurse will:

  • Complete your health risk assessment
  • Check your vision, blood pressure, height and weight
  • Evaluate your current health
  • Review your prescription medications and supplements
  • Answer your health-related questions
  • Find out if you are up-to-date on health screenings and immunizations
  • Develop a plan to prevent disease and improve your health

Follow up care will depend on the results of your annual wellness visit. Your nurse may provide you with educational resources, arrange further medicare-covered test or services, or schedule an appointment with your primary care provider.

What to bring to your visit

Since the registered nurse will be asking you about your family medical history, it is a good idea to write down any important facts you can think of before your visit. any information you have about your own medical history will also be helpful. In addition, please bring:

    • A list of all medications and supplements you are currently taking, including calcium and vitamins (include the dosage amount and frequency.)
    • A list of the doctors or other health providers you are currently seeing.
